Cardiac Tamponade Caused by Compression of Right Cardiac Chamber: a Hidden Enemy

چکیده
DOI: 10.5935/abc.20140195 Figure 1 – A) All reasoning to explain the cardiovascular collapse was concentrated on cardiogenic shock, as the serial radiographs were not suggestive of cardiac tamponade. B) The diagnosis was established by transthoracic echocardiography, which often does not disclose the diagnosis and makes the transesophageal echocardiogram a mandatory procedure. There is an image suggestive of an intrapericardial collection (arrow) causing right atrial compression. A 58 year-old male patient submitted to mechanical valve prosthesis implant was hemodynamically stable at the postoperative unit and over time developed progressive cardiovascular collapse, requiring high doses of vasopressor amines. The image illustrates selective external compression of the right cardiac chamber, mainly of the right atrium, which caused an unsuspected cardiac tamponade.

Right Atrium Laceration with Pericardial Tamponade: A Rare Presentation of Blunt Cardiac Trauma
Cardiac laceration from blunt thoracic trauma is not a common presentation. The rate of mortality due to this injury is very high since it is not diagnosed and treated immediately. In this study, we present the case of a 65-year-old man with blunt cardiac trauma, causing right atrial rupture and pericardial tamponade. Successful management of this patient was firstly done with initial pericardi...
متن کاملSurgical management of cardiac tamponade: Is left anterior minithoracotomy really safe and effective?
Objective: Cardiac tamponade is a life-threatening clinical entity that requires an emergency treatment. Cardiac tamponade can be caused both by benign and malignant diseases. A variety of methods have been described for the treatment of these cases from needle-guided pericardiocentesis, balloon-based techniques to surgical pericardiotomy. The Authors report their experience in...

A patient developed acute, severe hemodynamic deterioration five days after an aortic valve replacement. Cardiac catheterization revealed a markedly elevated right atrial pressure but a normal right ventricular end-diastolic pressure. Angiography revealed an extrinsic mass causing compression of the right atrium and the tricuspid anulus. A large clot overlying the right atrium and ventricle was...
متن کاملPredictors of tamponade and constriction in patients with pericardial disease undergoing interventional and surgical treatment
OBJECTIVE The aim of our study was to define predictors of cardiac compression development including clinical, electrocardiographic, echocardiographic, chest-X-ray and perioperative parameters and their diagnostic value. METHODS Overall 243 patients with pericardial disease, among them 123 with compression (tamponade, constriction) and 120 without signs of compression were included in the stu...

In any patient with a history of penetrating thoracic trauma, cardiac injury must be kept in mind. Here, we describe a 36 years-old female referred to this hospital with severe chest pain and hypotension. After primary evaluation and suggestion of AMI, streptokinase was started for the patient and because of deterioration of vital signs, cardiac surgery consultation was requested. After perf...
